Where Arhats presented a spare triangular composition of three elongated figures, One With Four Eyes multiplies the cast dramatically: a crowd of grotesque, distorted arhat figures — bald, wrinkled, bug-eyed, variously alarmed or beatific — press together in a composition of remarkable density, their robes in blue, white, and ochre, their expressions ranging from comic terror to transcendent vacancy.

The title — One With Four Eyes — singles out the central figure whose most distinctive feature is a face with four eyes: a traditional iconographic marker of superhuman perception in Buddhist art, the capacity to see what ordinary beings cannot. In Murakami's rendering, this attribute becomes simultaneously devotional and grotesque, the extra eyes bulging rather than radiant, the supernatural vision expressed through the visual language of Japanese caricature rather than sacred painting.
